Who we work with
The idea of empowerment might seem strange, but we all
self-manage every day, whether we call it that or not. We take decisions about what we eat, how much we exercise and rest, and whether to take our medication.
Health Care Professionals
A number of models of care now recognise that to get the best outcomes, patients and professionals need to work in partnership. This is generally referred to as person centred care.
Originally health services were organised to respond to and treat short episodes of acute illnesses, but today they are increasingly challenged to accommodate the long term ongoing care and treatment of chronic disease.
People are sociable, yet in many countries modern life, working patterns, affordability of housing are all reducing the quality and quantity of social relationships. Many people in these countries no longer live in extended families or even near each other.

For our first edition we thought we’d remind our readers of where we’ve come from by providing a timeline for what we’ve been up to since the Robert Bosch Foundation agreed with EHFF to fund the three year project.

With the disruption caused by the Pandemic our funders, Robert Bosch Stiftung have granted to all of their current projects some lee-way in their expected completion dates. We have decided to aim for end March 2021 so that from April we had a year to get everything up to speed.
